The Birth Information and Tracing Act 2022 provides eligible persons access to the full release of their birth certificate, birth, early life, care and medical information.

If you are a relevant person and you wish to apply for your information via the Tusla portal online please click “Apply for my information online” below.

If you wish to apply for your information via post, please print a copy of the form below and post to P.O box 13018, Dublin 15.
