STEP 2 – Complete the R1 Application Form

In the R1 Form, you will be asked a number of questions.

To ensure your application is complete, and to prevent delays in processing time, it is important to take time to answer the questions accurately.

Here are some important tips for completing the R1 form for home education:

• Answer ALL questions relating to home education.
• Enter your child’s full name on the application form, exactly as it appears on the certified photocopy of their birth certificate or passport.
• Ensure the PPS Number (Personal Public Service Number) of the child is present on the application form in Part A, Section 1.
• Ensure the name(s), address(es), phone number(s) and email address(es) of all legal guardians* of the child are present on all applications in Part A, Section 2. *Please refer to our FAQs if you have any queries about guardianship.
• In Part A, Section 5, if your child is still attending school, please provide details of the school and the year level.
• In Part A, Section 5, if your child previously attended any school, please give the details of the last school attended and the date they left and the year level.
• In Part A, Section 7(b), please include the address of the place(s) where the education will be provided, and enter the name, and other details of the education provider(s).
• If the child will be educated using an online programme, please let us know by filling out Part A, Section 7(c) and including the address of the online provider.
• Provide as much information as possible in all sections of Part B. All sections must be complete. Insufficient information will result in the R1 Application form being returned for additional information
•  All legal guardians* of the child must sign Part C of the R1 form. *Please refer to our FAQs if you have any queries about guardianship.
