The Mother and Baby Institutions Scheme is administered by the Department of Children, Equality, Disability, Integration and Youth.

If you were a mother or child in a Mother and Baby and/or County Home Institution and you do not know which institution it was, you may submit a Data Subject Access Request to the Department of Children, Equality, Disability, Integration and Youth.
How do I submit a Data Subject Access Request to the Department of Children, Equality, Disability, Integration and Youth?
If you would like to access your personal data which may be held in the records of the Mother and Baby and/or County Institutions investigated by the Commission of Investigation, you may submit a Data Subject Access Request Form to the Department and email the form to the following address: sar@equality.gov.ie

You can request a copy of the Data Subject Access Form by emailing the Department at: sar@equality.gov.ie or by visiting their website, where you can download a copy of the form.
If your request relates to an Institution which is not included on the list of the Mother and Baby and County Homes Institutions investigated by the Commission of Investigation, you may wish to forward your subject access request to the organisation which is relevant to your particular situation.
