Statutory Health Surveillance Assessments
Statutory Health Surveillance Assessment is provided by the employer under Section 22 of the Safety, Health and Welfare at Work Act (2005), Tusla are obliged to provide health surveillance assessments to support staff in their roles within the agency. Such assessments are carried out by our Occupational Health Service.
To avail of:
- Baseline Vision Testing: For employees who must use a VDU (excluding laptops) for continuous periods of more than one hour per day. Should an optical report state that glasses are required solely and specifically for VDU use.
- Night Worker Assessment: Night work refers to work carried out during the period between midnight and 7 a.m. on the following day.
- Audiometry: For employees exposed to a daily or weekly average noise exposure level of 80 dB or a peak exposure value of 135 db.
- Respiratory Questionnaire and Lung Function Testing: For employees who are likely to work with and be exposed to respiratory sensitisers.
The Statutory Health Surveillance Form must be completed by the referring line manager and submitting it to Central Admin centraladmin@cognatehealth.ie , please ensure to check names and email addresses are correct on the form.
